Neighbor of mine has a JD 750 and he pulls a finish mower behind it. When he turns the PTO on the tractor will not drive. If the PTO is disengaged, it drives slowly. Is this a burnt out clutch disk?

To possibly be of some help, I'd need more info about some history and what things he or you have checked out. Maybe others can pinpoint it without more info.

I took the 750 for a test drive today.

Here is what I noted:
1. Clutch pedal has a lot of freeplay
2. Low Range - all gears will drive tractor. Engage PTO with mower on and in any gear, tractor will not move. PTO shaft can be seen trying to turn.
3. High Range - all gears, tractor hesitate to move but slowly starts moving. Engage PTO and nothing.
4. Low Range and 1st gear in 2wd with turf tires - pushed into a pile of dirt with loader and it got about a half of bucket and stopped moving, no tire spinning either.

Are all of these symptoms of a worn out clutch?

Sure sounds like worn out clutch to me. The pto is not dual stage, so it is adding to the load on worn out clutch.
