Was my wall I put in primary mud soup drain behind first top course of blocks. Maybe perforated black coil pipe.
Best to get rid of water dat become mud soup fast as possible, not let soak down to bottom of wall.
Picture look like you got lot of future mud up above wall. Mud real sneaky works wid gravity to plug up drains over time.

Soil barrier above gravel and below topsoil?

I'm guessing that gravel means crushed stone? Then yes you want to keep the top soil (dirt) from getting into the stone...
