Need affidavit form ..QUICK!

My PCatty software on my work computer has crashed. I'm trying to locate a generic affidavit form that i can then customize.

I've got a meeting to go to in about 20 minutes that I need this for for! Anyone got any helpfull ideas /forums/images/graemlins/shocked.gif
Even if I can get an email.. or some cut and pasted text.. I'll worry about formatting and fonts on my side.

I have re-installed on a different PC but can't find an affidavit.
Any idea where it might be in the sample documents or a different name for it?

I'm not sure.. mine won't even load.

I kow I've had affidavit forms before.. I had assumed I got them from pc atty.. perhaps I am mistaken??

Well.. drawing on my years of eng. school ( no help ) and picking thru my notary handbook... I've hashed something together.. The boss literally grabbed it out of the sheet tray on the laser printer as it came out.. then hopped in his truck anbd left.... hope it works for what he wants! /forums/images/graemlins/wink.gif

Well, now that the fire is out, what happened to your software?
When I start mine, I'm getting an operating system error about running a 16-bit application. Same on my other software (Kiplinger's H&B Attorney).

So... I loaded both sets of software on a new computer but couldn't find an affidavit form on either?!?! /forums/images/graemlins/confused.gif

I did e-mail you with a template I found on the internet.
