Stx 375 transmission code

I am having a problem with my STX 375. Whenever it is in drive the tractor will go back into neutral automatically. I then have to put it in park and back into drive and after going about twenty yards it will do it again. It started to throw a transmission code (56004) and it says that the measured transmission output speed does not match the commanded transmission output speed. What could be causing it to throw this code and what could I do to fix it?

Is the speedometer working? I would check the output speed sensor.
