Oil & Fuel header tank

Has anyone been having leaking problems with the lower hose on Yanmar 1610 header tank? I've tightened the hose clamp and added another clamp behind. but still seeps out top of hose. I replaced header tank with new one and new hose, but still can' stop the seeping out of the top. As soon as you rev-up the engine, you can see the fuel seep out. Any ideas out there???...thanks
