Front wheel self-destructed

Has anyone else run into this? One of the front rims on my Kubota BX25D delaminated, and cracked through one of the lug bolts. I have been careful keeping the bolts torqued right, because the one that broke is itself a replacement (last November) for the original rim. On that one, I let the bolts get loose, and the hole became oval.

What do you mean by delaminated?
