Not positive, because I don't know all the facts, but looks like driver error. Looks like the boom was in the air while hauling. I have yet to understand why people won't stretch the boom out and down. Don't know why but the doghouse looks to be totally destroyed. Possibly landed on it before falling on it's side.

Nothing really being done wrong in this picture - just thought it was kind of funny that his car was so small that he couldn't fit it inside snd had to strap it to the back to carry it.

Gee, as if station wagons or darn near every Subaru and SUV ever built didn’t have a rack on top for just that purpose?

Not a very big excavator. The boom must have been up. Unless it was being hauled in some taller vehicle, like maybe the box of a dump truck?

Isn’t hitting a bridge always driver error?

My cousin in WY posted that yesterday on fb. It seems that it would be money ahead to install electric eye over height detectors a mile or so before overpasses so it would set off some train crossing lights or something to give a hint that something's amiss.

They have those exact systems in places with known low clearance locations that keep getting hit. I would assume it reduces the number of bad incidents, but I know it does not eliminate them....

In a nearby city they suspended a 4" I beam on chains in front of the low overpass. It gives plenty of warning, you'll know it when you hit that...

Fortunately he had 3 medium (at best) strength bungie cords holding it to the trailer

You think a driver in a semi tractor is going to hear that?

We have a bridge here which is narrow and has a posted weight limit.
They have a big sign with chains hanging down about 20" from the first bridge horizontal.
Amazing how the big rigs get right up to the chains before they figure out what the sign says.

Watching a big rig with a 53 trying to back up in a small intersection takes quite a bit of time.

Got a local low RR bridge that trucks get literally stuck under. They have all kinds of signs, hanging things to warn trucks, yet they still go down the road.
