Just had my whole driveway paved a month ago and got 4 estimates. Maybe different type specs get used in different regions but in my area all specified the following on their estimates:
Regrade area - add a base of 21-A crush run stone fine grade, compact and treat with weed killer. Pave with 2.5 inches of 9.5.
Two of the contractors said whether I chose them or not, in a year I should get it seal coated and then every 4 to 5 years thereafter. Doing so supposedly will keep it from deteriorating and last much longer.
