I think it was a City owned parking garage, City owned truck and a City employee. Workman Comp maybe. City's do not usual operate or allow paking garage to operate with holes in the roof. Sounds more like cost cutting sub-par construction or improper specifications allowed the decking to be constructed to not support the weight that was placed on it.

I came close to that scenario the other day. We have recently purchased a new property, not quite familiar with all the nooks and crannies yet. I backed my BX2660 into a hole while plowing, left rear wheel went into the hole and the front wheels came off the ground. I had to remove some weight from the ballast box, then I placed two cinder blocks on the floor behind the drive pedal. This gave just enough weight to right the tractor and pull forward out of the ditch. Still a tractor newbie but learning I guess.
