A farm I used to deliver feed to a BIG 1200 cow dairy far jus tput up a new 5 million dollar complex with all state of the art features. It had an automatic alley scraper on a timer witha cable setup running the full length of the barn 750 feet. The owners son was doing some maintaince on the unut it was in the off cycle, He left a cresent wrench on the floor under the unit , the emergency button was 5 feet from him, being a strong 20 yr old he figured he could reach in and get the wrench wellllll the unit kicked in just as he reached in and the cable caught him he couldnt get out the cable cut him in half and it didnt shut off one of the other workers had to hit the emergency butto to shut the unit off. 3 hours it took the paramedics to get his remains out . The moral is the shut off switches are installed for a reason.

Lockout and Tagout, Always! Even when working at a very safety concious IBM Fab, a worker was killed by a robotic arm. Even the "fail-safes" failed but would not have been an issue if circuit had been locked out and tagged out.
