Can you even get to the bottom of it while it’s in the truck?

The newer models are installed from the bottom, so yes. And the older style with cross bars was also accessible from below.

And it sounds like the OP did attack the ball from the bottom, just with the wrong tool for the job.

I should dig up photos from one of the many B&Ws I have installed and refresh my memory about what the very top portion of the hole in the receiver looks like.

Haven't messed with it much. It's not really too much in the way and with the nicer weather, been real busy.
