Rake - what model?

My rake has rust so I cannot see any manufacturing or model info. By any chance, does anyone here recognize it? I need the make/model number to get some parts for it.

Welcome to TBN . What parts are you in need of ? Looks like anything on there can be matched or fabricated if need be .

Both tires, one (or both) tire rims, top pin is missing (see photo), and side connection to 3-pt hitch needs to be replaced too (doesn't stay tight, threads are messed up)

Tires/wheels should be simple, all you need is axle diameter, wheel diameter and width and then a tire size which should be obtainable from side of tire or a tire shop should be able to measure the tire width fairly closely. Wheel diameter will give you the tire diameter.

Top pin is also simple all you meed is a diameter of the hole. Not much to go by in the picture for comparison, but a guess would be 3/4” or 7/8”.

As for the side connection for the 3PH, use bolts, nuts, washers, lock washers and possibly some thread locker. You may need to run a same size drill bit as the bolts to clean out some threads for the bolts depending on how badly damaged the existing threads are, you may be able to use them as part of the entire bolt process.

OK, Thanks! Knowing the make/model would still be nice, maybe it can get me expanded diagrams of the whole thing. It was given to me for free, and has been just sitting outside for years. Should I disassemble anything to grease it? Like the columns that hold the wheels, its hard to tell what I should do to it to grease/restore it.

I had a rake nearly identical to this - I believe its a York Rake - an older one. As Coy says all the parts you can get at a Tractor supply.

The 3PT connection pins with threaded end are a few $, and the angle stop pin probably 7/8" x 5" and just do tubes in the tires will be easier and cheaper. Or get run new run solid wheels - probably a 1/2" or 5/8" center bolt.

Yes parts from TSC , Rural King etc . Not sure who made your york rake but it could be a vintage actual York Rake Company made back in the day in Umattila NY . Just a wild guess because of design and fadeing paint color ...............

That's Unadilla,NY. Had a friend that did drafting for them as a summer intern his third year of college. We lived about 20 miles from there.
