Ive had some similar problems. Usually once per year I take a wire "toothbrush" and some WD40 and clean up both the male PTO and the female PTO coupling. Then wipe it down and visually inspect it very carefully. If a little ding or burr is found, then dress it with a file or emery cloth and you'll be fine. OBTW, a good pair of reading glasses are helpful, if you're as old as me...!

This may or may not help.....
My tiller was a bear to get on. Then I got Pat's Easy Change, and the shaft slipped on very easily. I thought about it for a minute, and realized the shaft was too long, and PEC extends the implement out by 4 inches or so. So I solved a problem I didn't know I had, in addition to making the 3ph attachment easier.
