DIY hv5902 adapter plate

Help anyone? I am wanting to build my own HV5902 adapter plate but I don't have enough information to proceed.

I need to identify which ports go where. I have a couple of pictures of the plate, see attachment, but not sure which hole connects to each hole.

I have all the tools and equipment to build one, mill, lathe, taps etc.

If someone has a plate they could loan I would pay shipping both ways. Or if someone could take detailed pictures I could pay a small fee for their trouble.

attached is another picture with several views of the same block.

I think the lines shown in green are hole connections to the ports.
The RETURN port i am pretty sure is correct.
The PBY port may be correct.
I have no idea on the PRESSURE port connection.
