We had bunk beds. The mattresses sagged a fair bit in the center because of the way it was supported with steel straps and springs. It sort of created a hammock effect. I always figured that was part of the design to keep you away from the edge. I don't recall anyone ever falling out except the time our dog heard a noise in the middle of the night and jumped off the top bunk to investigate. Bark, bark, thump.
