I'm at a loss to understand what happened?

Was it an attempt to set the bead with ether?
Or over inflation pressure?
I don't understand how a ruptured inner tube could do this...am sure I need more education.

Anything that kills one and maims another is worth understanding and avoiding repeating the problem.

Sorry to hear this happened to anyone much less people you know.

since it mentions an inner tube.. then it couldn't have been an there -= bead setter, as with an innertube, you just soap the beat and casing/rim up with tire soap and then air up the tube.. if it don't seat perfectly. you overinflate it a HAIR.. if sill not seated. you deflate and try it again and whack the casing with a hammer near the area it don't seat.

finally if still no joy you can install the rim and rive on it a bit and they usually seat with a pop pretty fast.

depends on tire size, load range, ply # etc.

100 on up to 120 not uncommon

I found another report...copied below...does not shed any more information on the subject other than it was a terrible event. There must be more to the story of how a burst inner tube caused an explosion.

According to Tishomingo County Coroner Mack Wilemon, Chester Marsh, 46, died about 3 p.m. in an explosion at Parkers Tire store.

He said Marsh was mounting a tractor tire on a rim when the inner tube burst and caused an explosion. He said the explosion picked him up, destroyed a 10-foot garage door overhead and destroyed a 12-foot ceiling overhead.

Wilemon said an Itawamba County man was injured in the incident and was treated and released from the Red Bay, Alabama, hospital.

Could they be describing a split ring hub?
They used to make, even pick up truck rims, with a split ring on one side. After the tire was put on the metal split rim ring was put on and pried under that side of the rim to make a tire lock when air was put in. It was extremely dangerous as the ring, if not put on just right or if there was rust, could be blown off and do tremendous damage. Tire shops had cages that they used to put the entire unit in prior to
airing up. I had a 70 something Chevy PU with them and changed a tire once. Too dangerous. Took them to town after that. They were outlawed on pickups later. Don't know about semi tractors.

yup. and if you didn't have a cage, you put chain around the tire and rim in a couple places.

I hate split ring rims..

We used to bring the lift down so it set just above the rim, and used a long clip-on chuck. Still hated doing split rims.

Small tractor dealership I used to work at would weld the split rings before inflating. Those tires were used on the forklift. When they needed repair, way cheaper to grind the welds off and reweld after repair than to cause an injury/death if the ring were to fail.

I have mounted many many split rims in the past. Even without a cage you hook up the air and stand back. Control the air flow by a valve so you don't get over the rim.

I will hazard a guess the air hose was clipped on the valve stem and left to fill till the rim popped. If the air compressor was set to 200psi disaster was imminent.

So sad it happened.
