Machinery safety is like gun safety. Have rules, enforce them, and convey that they are never, never, never to be broken or consequences will be severe.

The kid is just as dead as if he were allowed near an unsecured weapon which was charged and had the safety off, and chose to play with that.

Orders are orders, and fair discipline is one of the most important things you can give any child.
