This happens only occasionally, and has been happening since the ECU was replaced chasing a Regen issue. Any idea what the code means and the cause?

Looks like your dash is doing a second power on self test. I'd look at the dash connections and re seat. Ground and power to the cluster. I expect a bad ground connection. It could even be in your ODB connector, had that on a jeep....
Does it only do it on start?
Also make sure your ECM cables are not rubbing on the radiator support.

On mine the temp gauge goes to wacky. 80% works great and correct, 10% it does not move at all, the other 10% it stops random places on the dial after power on self test. It even goes all the way past Hot and around to the stop pin... then tries to display from where ever it stopped. Sometimes a restart clears it other time only get worse. I added a block temp gauge and it ignore the LS gauge when it wants to be stupid. Will work fine the next day, it is in the cluster and not worth 250 for me to swap the cluster and too fiddly to look at until after I have eye surgery next month.

I would not be shocked if these type of things are cold/bad solder joints on the cluster.

Your dash only showed 11.6 volts even before you started cranking it. It's normal for the system voltage to drop from around 12.5 volts with a good battery to around 10 volts while cranking because of the draw from the starter. If it drops below that, the ECU, VCM, and instrument cluster can act weird from getting low voltage and will cause your dash to go through another self test after starting. It can also cause a crank no start and trigger fault codes if it gets bad enough. They sometimes sound like they are cranking over fine so it can fool you and you may not suspect low voltage or weak battery while cranking. Put a volt meter on the battery or starter connections and see what the voltage drops to while cranking. Mine cranked just fine but the voltage dropped to 8 volts on a cold morning causing a crank no start situation because the VCM stopped working below 9.2 volts.

low voltage was mentioned a while back when I posted about just the cluster reset, no video and not mentioning the code. At the time I put a battery tester on it, and it tested good, it has been a while, so I'll do it again and report back.

I put a meter on it, was 12.8v. CCA was a little low, 513CCA and rated for 660CCA
I have it on a charger now. I remember last time I dug into this problem, having a fully charged battery made no difference.

I think the glow plugs were just dragging the voltage down in the video
