matching yoke numbers?

my yoke splines stripped out on my hydraulic motor shaft. I can see several numbers on the yoke, but I've googled and can't seem to match anything close. The yoke is stamped MADE IN USA, so i thought it'd be easy.

Under the made in usa, it looks like "F7" or F1 is stamped. Then below, it has 0502-AC, under that: MFC 17, and under that: D6S.

I've tried google searches with all those different combinations and got nothing. It is a 13 spline yoke.
