JD455 Blower Shaft Ext Slipped out...wt...?

Wow, I made a mess of the shaft extension on our first snow fall. Shaft slipped out somehow - lifted too high maybe? - and slapped around on the QH plate chewing up the ends of the teeth on the shaft.

I got lucky with a little filing work and got her sliding back in nicely on the second attempt at cleanup.

Just don't understand how that happened. Any advice to prevent in future would be appreciated.

I took some measurements before/after re-install and based on the numbers in a variety of positions, I can't see how this happened. Shaft is 4.75" at teeth and insert is roughly 1.6" (1 5/8"). The slip connector to the long shaft was still intact and firmly seated.

Just unusual bad luck or something I'm doing wrong.... Thoughts?

I presume that you put the u-joints back in phase(time)? According to parts photo shaft has a timing pin. I think it might be wise to put a hyd cylinder stroke limit on snow blower so this won't happen again.
