When I was building my suspension bridge, I was using a come-a-long to adjust height of deck of bridge. I would try to take into account what might go wrong, as I was suspended over the water about 15', and over the land about 10'. I would lay down the floor made of expanded metal until I run out, then would work off of a 2x6 so I wouldn't have to stop working when I run out of material and could keep extending the framework and cable assemblies. Well, something I hadn't planned on was the plastic handle on my come-a-long slipping off. I had re-adjusted ropes, chain, etc, positioned my board in the place I was going to sit, and grabbed the handle on the come-a-long to ease myself into position to sit on board, when the handle slipped off. I continued over backwards and just happened to hit the water. I usually always expect the unexpected, but this was a complete suprise. I didn't get hurt (this time), but had I been over the land when I hit it could have ended differently. Glad to hear you landed safely - but it is pretty funny.

Long time ago working in a factory I was helping a friend to tight transmission idle shaft. The wrench gave in and I flew in a box of gears; they really hurt your butt and back when you land on them.
