If this herbicide residue is the issue, how do I fix it? The link dgeesaman gave, gave some other links, but all described the problem but not the remedy.

I am no expert, but with that out of the way, I can say I had the same experience- old, super composted black manure from a hillside. I was told that it has too much nitrogen, so check for that. Made for real leafy tomatoes, but not much fruit.
The chemical residue may be harder to check for...

Will weeds or grass grow in your garden ?? Every put any lime or fertilizer on it when you planted it. Do a soil test and see whether it's just low on N,P or K .

I am glad my garden soil is good enough to not put anything down on it.

ADD ORGANIC MATERIAL!

The solution to pollution is dilution!

Too much nitrogen? Just add compost (always a good idea) and let some time pass. It's a temporary problem, I think. Now, nematodes and tomatoes are another issue... Gotta move the planting area around a bit from year to year. As CalG says, organic matter is the berries for gardening.
I mentioned this thread to my gardener-scientist wife, and she said that after 3 years, it probably isn't excess nitrogen. Soil testing is called for, I suppose. Good luck, and let us know what you discover, please.
