Perhaps to avoid that nut falling off, you could swap the standard nuts out for some locking nuts. Or even use the appropriate locktite type to keep them on...
I was in a hurry on a project and omitted the lock washers and locktite and locking nuts, not a problem, I'll get it later.. the other day, I noticed one of the nuts was missing, replaced it and remembered why I didn't use one the first time (I am out of that size!)....
Nice workaround, should last forever.
