Without reading the entire document motor vehicle crashes seem to be number one in cause of death. Could it be that urbanites died while traveling in rural areas? The rural hospital next to a ski resort will have a high incident rate of broken bones compared to an urban hospital. One has to look at the information in context. You don't get many drownings in mid town Manhattan, yet its the number two cause of children's deaths in rural areas. Several vacationers have drowned this year in local lakes and rivers, PA Pocono mountains.
