Planters Can anyone identify this planter

This planter has been in my family for many years and I'd like to get a little info on it. Can anyone identify it based on the pictures?

I've decided to plant an 1.5 acre corn plot this year so I pulled her out of the weeds and after a few hours of cleaning, oiling and greasing, it's rolling like a champ.

A few questions:

It currently has 614A corn plates installed. Is it possible to use the same plates to side dress the corn when it's time? I have the results of my soil sample.

Is there such a thing as a fertilize plate for this planter?
