Leejohn is the winner!!!
It was nut and washer 140&150.
They were missing and the intermittent was caused by the bolt staying in there, but with too much play.
I eyeballed it larger than 1/4. I had a 5/16 course nut that turned on it, but bound up. I went to that hardware store and tested my nut on the 5/16 fine and the metric 8 or 10 1.25. I really thought it was going to be metric, but it was 5/16 fine/24.
Leejohn is right, not much room in there, so I took off the hydraulic filter and was able to do it.
Put everything back together and reverse is back strong.
