I'm not a horse person, but I'd imagine you wouldn't want to follow your natural instinct to jerk your foot away, as that might startle the horse and cause it to rear, therefore putting more pressure on your foot.
My cousin in WY is a horse person, and an OR nurse so she is pretty much immune to gore that would make the rest of us blanch. She posted a photo on why it's not a good idea to wear flip flops around horses.
If you try to pull it out he will put more weight on it. Push or lean on him & he will lift his foot to maintain balance.
How do you hold your hand when feeding a horse sugar cubes or grain???
