For what that's worth, pretty much every one of those links is a regurgitation of this one, single report:

If we're to believe that article, fully 35% of all hospital deaths are caused by medical mistakes. 35%.

I get it...but the point is...mistakes are made some monumental...When it comes to health care consensuses are the norm before procedures are adopted...some turn out to be wrong...
