There needs to be a locking arrangement on the lever assemblies to prevent unlatching if working in a forest environment like I used to do recently. I can visualize it coming unlatched by an errant branch.

This QA style has been around since the 90's and you would think someone would've already implemented a true lock for the SSQA mounts.
We added protective bars to the back of all Wicked Grapples a while back to help ward off stray limbs after seeing this video from Eric.

Had the grapples come off once in the fire, now run a chain around the cross tube on the loader and through the grapples and secure with a clevis so they will not come off the QA even if the levers get flipped.
