E wheel. Why is the left rear deck wheel one hole different from the others on my GX345 ? The left rear has four holes showing and the other three have three holes showing. I hope it is supposed to be that way and not that the deck hit something and is tweaked. I need two front deck wheels, should I get the ones with a grease fitting or is it not necessary ? I could just grease the shaft periodically by removing The wheels, it is only one nut to remove them.

move the one with four so it has three showing

Simple as this.^^^^^^^

They are adjustable and should be approximately 3/8" - 1/2" off the ground when parked over cement or similar surface with the mower deck in the cutting position. All of this is in your ops manual. If you don't have one, get it from the dealer.

Make sure the tire pressure is right so the tractor is level and then park it on level concrete. Then set the mower the right height and set the wheels a little bit off the ground. Make sure the deck is level side to side and front to back. Consult your owners manual to make sure of that. But, hopefully they would all be in the same hole.

I like wheels with grease zerks. I would be more apt to grease them otherwise I would just overlook them..

when measuring from the concrete to the top of the top of the hole where the gage wheel shaft goes, there is a difference between the front and back of the deck. If I put them all with 3 holes showing, the rears are farther off the concrete than the front gage wheels. Is this right or am I missing something ?
Also it says in the manual that the rear of the deck should be 1/2 inch higher than the front of the deck. Am I reading this correctly ?

Yes, that is fine. Don't forget about the pitch of the deck. Trust me, 3/8" on the blades is what you want, and besides, that would make the deck pitched about 1/2", since the deck is wider front to rear than the blades are long, and pitch should be set on the blades on a JD.
