I salvaged a set of forks and the mount from an old IH forklift which was about to meet the crusher. I ordered a set of quick attach brackets for my Westendorf loader and bought some metal off of a local shop. Pictures will speak for the rest. More to come as I get time to put this together.

I have about the same thing. I cut off all the extras, wheels, chain, etc. I also welded a 2 in receiver at the bottom for towing, pulling, etc. I welded it far enough back so as not to interfere with the forks sliding. If you take out the bolts on the ends, you can slide the forks on in the center or from the ends.
