"Analysis of accidents" another way of saying, Hindsight is 20/20. I had my share of near misses and that's where my 20/20 works great, should done this instead of that.......probably 90% of accidents are caused by mistakes but at times when I'm cutting firewood, seems like the darndest things happen.
Some of these story's here to me is attention getter, and if someone tells me that they don't ever make any mistakes, I wanna see em walk on water.

Really unfortunate and a good reminder for the rest of us. I think the pot is the scariest part of the tractor even shielded

Hindsight nope. Everybody knows PTO should be shielded. Everybody knows don't wear loose clothes around rotating machinery. Everyone knows keep your distance from machinery that is working. No hindsight needed and no mystery here although that does not diminish the tragedy.

That's why I like ice fishing.... get that walking on water feeling. :D

But in all seriousness, that's a dangerous occupation, too. Good ice here, bad ice there. Take your best educated chances.

Yeah, you'd think, but I never new any of that until a farmer friend told me, having never been around rotating machinery before. If you never seen it, or heard of it, or been exposed to it, you don't know it. I'd wager a majority of people don't know about the dangers of rotating machinery.

(Everybody does not know), I didn't know when I first started using farm machinery, don't remember that being taught in school, it was later on after herring a few horror story's that I learned about PTO danger. People like me that are not born with a 2000 IQ have to learn as we go and some of us learn the hard way. The salesman who sold me a tractor, reminded me why this PTO has a plastic guard on it that runs my winch and that guard will stay on after reading some of the story's on this thread. Reminding people/me of the dangers and hazards of moving machinery is always a good thing.
Now I'm not a safety guru by no means and or a safety cop, so if someone removes a safety guard on something, one must pay extra attention when using equipment.

Ok I grew up with machinery. So it is second nature. Even so I look at the circumstances of any accident to see what happened and learn from it. "Analysis of accidents " is not academic or casual hindsight. The alternative is to say "stuff happens" and just plan to be lucky.

I read several posts that stated the PTO drive shafts should have the shield...very true!
But never depend on that shield!!! the bells on each end of the shield don't extend enough to completely shield the yoke and u-joints.

The only safe way is to keep one's distance...nothing else is 100% safe

Kind of like the safety on a gun, it is a good thing to have, but the real safety is between your ears. I always have and always will keep the PTO shields on my PTO shafts. Yes, they can be a PITA sometimes. But they can help if you mess up.
