Need help identifying 3ph Winch

I need help identifying this logging winch...it needs repaired but I have no clue what kind of winch this is. I have looked over the entire thing for a part # or model# and cannot find anything.

I am thinking there is a strong possibility that it is Home Grown. Can't be sure of course but I have never seen one like that. It does not look like factory welding or fab work to me.

I'm pretty sure its factory...except for 2 eyes that were welded on from someone, because on the back it has a sticker that talks about making sure you keep it greased...first language is Italian, then French, English, and last German...so not sure.
