in my case, I use PT&G reamers.
Yep, Dave Kiff at Pacific Tool and Gauge Chamber Reamers, Headspace Gauges, and Firearm Accessories Since 1957 - Pacific Tool and Gauge
... Dave has a lot of my money
The closest custom rifle shop to me was Spencer Barrels (of Clay Spencer fame) Spencer Rifle Barrels ... I'd go hang out there and be like a kid in a candy store ... he had all the toys (at least 7 lathes, some manual some CNC, several high end mills etc.) even made/designed his own button rifling rig that was pretty impressive (I think he was on the 3rd or 4th generation of it). He would get blanks and do all the rest ... even had a custom double stacked kiln to stress relieve the barrels, expensive air gauge etc.. I'm more of a cut rifling kind of guy but there is no denying the success Clay's button rifles (and other's) are having in the competition world. They certainly are faster to make. But if given a choice ... I'll take a cut one off a Pratt & Whitney machine any day of the week
Clay retired and sold his shop to someone out West if memory serves me right. He even had a room full of swagers with people pulling the handles cranking out custom bullets too!
I've seen talk of saving old barrels and using liners but don't know anyone that does that kind of work. I'm sure the dudes at Military Surplus Collectors Forums would know if you're really set on it. The overall length of both cartridges (7.62x39 vs Grendel) is pretty close. Looks like there are some threads on weaponeer about what you want to do http://www.weaponeer.net/forum/forum_posts.asp?TID=6458
