I got a set of the TS quick hitch that go on the lower 3PH arms. I have been using them for a little while with no problems, but yesterday I was pulling my 72" Befco grader and 4 or 5 times one side or the other popped off the pin on the grader. Almost like it vibrated it just the right time and came out of the latch. It looms like there are holes to put a latch pin through but because of the outer casing of the QH, I don't see how you could get one in. Any one else have this issue?

Just a ? , but do you need shims / cat 0 to cat 1 , to go on box blade pins to properly fit the quick hitch ? If the pins dont fit corrrectly that my be why they are popping out .. Had to do that with all my 3pt equipment when add my 3pt quick hitch.

If I'm reading your post correctly, it sounds like you have something similar to a Pat's Easy Change rather then a Quick Hitch.
Does your "QH" attach to both your lower links and the toplink? Or, are there 2 components that attach to your lower links?
I looked on the TSC website, but that just shows the standard 3 point mount QH.
A picture would help, I think...

Yes I have the TSC version of the Pats hitch. I also have the shims on the grader so they fit. If you look on the web site and see the clasp that goes over the pin after yor raise the arms. That is where the problem is, I don't know how but that clasp is opening up and the grader is popping out.

I don't see anything like that on the TSC website, but I think the lower link arm is swinging out and pulling off the implement's pin.
The Pat's system uses a bent washer the lynch pin goes through to prevent this movement. I had a problem with my Pat's system doing what I think you're describing. In my case, it was due to the lynch pin spring loop not being strong enough to hold (mostly when turning, BTW). I got some of those black lynch pins with the stronger spring loop.

You'll also want to adjust the tractor's lower link anti-sway device (chains or whatever) to limit side to side movement.

Again, a picture would probably help...

Are you using a washer on the implement pin between the QH and the linch pin? That should stop it from coming out and overstressing or unseating the linch pin. It may also ease the job of the "clasp" holding the implement pin down in the U of the QH that seems to be causing you trouble.
