Have had my 3540HST/LA724 for 6 months but did not get an extra QA Plate cause it was 120.00$ and I figured I could make one myself.
Next I found the standard plate sizes, angles, and cutouts from a post here. But I have forgot the link[search for SSQA Plate sizes].
So I found a piece of 1/2" plate the right size as the manufactured size.
Lucky the plate was drilled and tapped for 1/2" in several places, which I re-drilled for 5/8" and re-tapped.
Mounted my current plow angle mount and added other 5/8" eye bolts so I could also use it for lifting logs to be bucked for the splitter.
P.S. Paint is from T.S.C. Val-spar Implement Orange [gray can @ 3.99], it's not exact but it's so close if feathered no one would ever know.
