It looks simulated. Combustion and exhaust dont look realistic.

I don't understand what you don't think is realistic.

Intake valve opens, as piston is going down, thus sucking in the fuel/air mixture. Piston rises on compression stroke, spark is introduced, and at TDC, combustion occurs pushing piston down. At the end of down stroke, exhaust valve opens as piston begins to rise, pushing out exhaust gases.
