1500D Wheel fell off!

The front left wheel on my 1500D fell off yesterday! No, the lugs didn't come loose. The hub and "axle" came out. The axle has threads on the end (apparently undamaged) and it looks like the nut just came off. I was at my girl friends and therefore didn't have any of the manuals or tools. So I took the wheel housing off and put everything into the pickup. After getting home, I looked at my manuals. I have the Japanese part manual and the IT shop manual for the US models. The part manual seems to indicate that there are two snap rings which may be just holding the bearings in place, a washer of some type, and an odd looking nut. I haven't taken it apart yet, but from what I can see, the bearings shaft, etc look good. The seal needs to be replaced.

Has this happened to anyone else? Is there anything I need to pay attention to? Any advice will be appreciated.
