He somehow NEVER looked in his mirrors??
Yeah, that's the thing I always wonder about these situations. I mean, I'm the classic "absent-minded professor", as forgetful as anyone you'll ever come across. I could easily see myself pulling away from a dump site with the bed in the air if I did this a half-dozen times a day, year after year... it's bound to happen.

But to get so far as to actually find yourself on a highway, with never having checked your mirrors and noticed the bed was in the air? Totally unacceptable. I must glance at my mirrors every few hundred yards!

If the PTO was still engaged, I guess it's possible that once he go going, that the hydraulic control got nudged ... But on the ones I've seen when I either worked on them as a mechanic, or drove them, they had a collar that you pulled up to unlock it, to allow you to move the lever out of the down position ...

However a similar incident happened a few years ago down near Macon, GA, which had a full load of gravel, and it hit a bridge!

I don't think that the car driver behind him is the sharpest tool in the shed either? I mean, your following a dump trailer with the box up in the air. Would you not want to back off?
I've encountered debris in the road, was able to see and straddle it. Yet the tool 20ft behind shining lights in the driver side mirror slammed straight into it. Sure was a way of backing people off, way off. Maybe the box up was useful after all.

Re P2312 ,bridge strike. WHY is it illegal to hang a steel pipe to act as a bell 100 yards out from an overhead bridge (space permitting)? This would acr as a NONdestuctive over height load stop. Instead they only put RIDGID beams DIRECTLY on the bridge to wreck the truck.
There was a bridge that was hit many times so they measured it. The height sign was WRONG.
The reason was the road just had overlays added and the height sign was never changed, so legal permitted overhight loads would hit it.
Sadly, all of this was done and they still hit it.
